By completing your booking you authorize ADX Diagnostic Lab to perform a COVID-19 Test, including collecting a specimen, which, depending on your test type selection, may require ADX Diagnostic Lab to do one or both of the following:

  1. Take either a few drops of blood by finger prick to perform the test or draw a small amount of blood by venipuncture (1/2 teaspoon) to perform the test. (COVID-19 Antibody tests)

  2. Take a nasal or nasopharyngeal swabs to perform the test (COVID-19 PCR, LAMP, and Antigen tests)

You should consult with your physician regarding your test results. If you do not have a physician, you should seek medical care and advice regarding treatment based on your test result.  ADX Diagnostic Lab is not responsible for interpreting your results or providing medical care or follow up.  As with any laboratory tests, there is a potential for false positive or false negative results.  Additional testing might be required if results are inconclusive.  ADX DIAGNOSTIC LABORATORIES IS NOT RESPONSIBLE OR LIABLE FOR ANY FALSE POSITIVE RESULTS, FALSE NEGATIVE RESULTS, DIAGNOSIS OR ANY OTHER INFORMATION.

Testing Procedure Discomfort:  The COVID-19 testing procedure may involve some degree of discomfort, and/or other negative reaction. You expressly understand, acknowledge, and accept that you may experience discomfort or other negative reaction as a result of receiving testing services under this Agreement. You further understand, acknowledge, and accept that this discomfort or negative reaction is inherent to the testing process and not a result of any fault by ADX Diagnostic Lab.  You agree that, except as provided by law, ADX Diagnostic Lab is not liable for any discomfort or negative reaction you experience as a result of the testing services.

Booking and Cancellation Policy: If you order your tests with ADX Diagnostic Lab and cannot meet your original appointment you can reschedule your appointment 12 hours prior your current appointment. You can cancel 12 hours prior your appointment. Cancellation should be done via email (customerservice@acceldx.com). If you cannot email and the lab is close or no one is able to take your call, please leave a message with your name and appointment time. If you don't cancel 12 hours prior your appointment or miss your appointment there is a non-refundable cancellation fee of $20 for administrative expenses.

Test Results Delivery: Your test results will be sent to you within 30 minutes (for antigen and antibody tests), one hour (for LAMP test), two hours (for RT-PCR test expedited), and between 8 and 36 hours (for RT-PCR tests), depending on your time of testing, and after the test results become available. In some cases, a test may need to be re-run and this may cause delay in receiving your test results. Result times are not guaranteed.  You will be notified that your test result is ready by email (we will use the same the email address you provided during the online booking). You will need to follow the instruction on that email to view and/or download your lab test result from our patient portal. In some cases, you may receive your test result by encrypted email, for our encrypted email adxlab@adxlab.hush.com with subject Your COVID-19 Test Result. Once you receive our encrypted email, please follow the secure link. Encryption is an important step to meet regulatory compliance (HIPAA) and protecting your personal health information

Use of Information:  ADX Diagnostic Lab may us the information that you provide us for our operational purposes and to contact you in the future.

Insurance and Government Coverage Programs for COVID Screening Test: COVID screening services may not be considered medically necessary by your health insurance plan or Government Programs, in which case your health insurance company of the Federal Government might not pay and you will be responsible for payment.  Please confirm with your insurance company what tests they will pay for. Your insurances or Government Programs may not cover COVID-19 testing for travel purpose. It is your responsibility to check with them prior to booking your test(s).

Disclosure of Test Results to Governmental Agencies: Federal, state, and local governments require laboratories to report the results of COVID-19 testing, whether positive, negative, or inconclusive.  You acknowledge and agree that ADX Diagnostics Lab may report the results of your COVID-19 test to such governmental authorities.  ADX Diagnostics Lab is not liable for such government agencies’ use or disclosure of your COVID-19 test results and identifying information.

Disclosure of Test Results to Employer: If your COVID-19 tests are paid for and provided by your employer, you acknowledge and agree that your employer will receive the results of your COVID-19 test, regardless of the results.  ADX Diagnostics Laboratory is not liable for the employer’s use or misuse of any data, testing results, personal or medical information, or any other information provided as a result of the COVID-19 testing services.  You expressly agree to waive any applicable HIPAA protections relating to the results of the Testing Services to the extent necessary to fulfill this section.

Certifications: By clicking below, you certify as follows:

  • You are the patient and are at least 18 years of age or you are the legal guardian of the patient and have the legal right to consent to the test on behalf of the patient.

  • You understand that the tests have not been approved by the FDA but have been authorized by the FDA for emergency use (EUA), and you have received a copy of the Patient Fact Sheet relating to the test.

ADX DIAGNOSTIC LAB IS NOT LIABLE FOR ANY CONSEQUENTIAL, INCIDENTAL, SPECIAL OR PUNITIVE DAMAGES, WHETHER FORESEEABLE OR NOT, WHETHER BASED ON CONTRACT, STRICT LIABILITY, TORT (INCLUDING NEGLIGENCE) TO THE FULLEST EXTENT PERMITTED BY LAW.
